Introduction

Client intake is a crucial process for a business so we want to make sure that the experience is simple and efficient for both you and your prospect.

B12’s product includes a contact manager, a simple CRM, that organizes your prospects and clients in a single location. All forms submitted from your website sync with the contact manager.

  • If your intake process requires your prospect or client to complete a form, you send your client a link to the intake form page on your website. After they complete the form, it will immediately display in your contact manager.

  • You can also store notes from your intake calls in your contact manager. This way, all of your contact’s information, forms, and notes are organized in one place.

  • This article will walk you through the process of creating, sharing, and viewing client intake forms as well as creating, completing, and viewing intake notes.

Creating a client intake form or note-taking form

  • Get started by clicking on Intake forms link.

  • Then click on the Add form or the Create a new form button.

  • This will create a new intake form page on your website. This way, you can send your prospects or clients to a direct link to you website rather than emailing a pdf or sending a text document back and forth.

  • Enter a Form title and select the Form type.

    • External form types refer to forms that you intend to send to prospects or clients.

    • Internal form types refer to forms that you intend to use for your own note-taking.

  • You can edit or add fields to your form under the Form fields section.

  • Update the Page title and URL by clicking on the preview box under the Search engine optimization section.

  • The Page title displays at the top of the website visitor's browser window or tab.

  • Then scroll down to the Page URL and and enter memorable URL for form (e.g., servicename-intake, client-intake, or servicename-intake-notes). For the example below, the URL will be www.website.com/immigration-intake

  • Click the Preview button to see a preview of your intake form page. After the website loads, add the Page URL from the previous field to view the intake form page.

  • When you are ready for your form to go live, click the Publish button. After the website loads, add the Page URL from the previous field to view the intake form page.

View form submission in the contact manager

As mentioned earlier in this article, B12’s product includes a contact manager, a simple CRM, that organizes your prospects and clients in a single location. All forms submitted from your website sync with the contact manager — client intake forms and note-taking forms. This way, all of your contact’s information, forms, and notes are organized in one place.

  • Here's the contact manager. You can see all of the contacts who have completed forms on your website, including client intake forms and note-taking forms.

  • If you click on the contact’s row, you can view more details about that contact, including all of the form submissions and intake note forms you completed.

  • You can also view all submissions for a specific form.
